The Diabetes Prevention Program can help you reduce the risk of developing Type 2 diabetes and help you adopt and maintain a healthy lifestyle to reduce your chances of developing diabetes. This evidence-based program—which earned full recognition from the Centers for Disease Control and Prevention (CDC) for meeting or exceeding quality benchmarks—helps individuals with prediabetes prevent or delay the onset of type 2 diabetes.

Our lifestyle change program curriculum is part of the National Diabetes Prevention Program, led by the Centers for Disease Control and Prevention (CDC). It is designed for people who have prediabetes or are at risk for type 2 diabetes, but who do not already have diabetes. The lifestyle change program is proven to help people with prediabetes prevent or delay development of type 2 diabetes.
The Program runs for a period of 12 months. This 12-month lifestyle modification program—which is offered online and in-person at Saint Francis Hospital—provides nutritional guidance, exercise sessions, and supports to help prevent or delay diabetes onset. Throughout the program, a trained Lifestyle Coach will give participants the help and support they need to make and sustain lifestyle changes to prevent or delay the onset of type 2 diabetes.
Participants receive tools to help them monitor activity patterns, eating habits, and physical activity to assist them in achieving success. Participation in the program is free to qualified registrants.
Eligibility
To qualify for this program, you must:
- Be 18 years or older
- Be overweight (body mass index above 25, or 23 if you are Asian)
- Have NOT been diagnosed with type 1 or type 2 diabetes
- NOT be pregnant
In addition, you must meet ONE of the following criteria:
Have received a result of high risk for type 2 diabetes on the Prediabetes Risk Test
- Take the CDC Prediabetes risk test
- Email your results to Trinity Health Mid-Atlantic at thma.dpp@trinity-health.org
Program goals
- Lose seven (7) percent of your current body weight
- Gradually increase your physical activity to at least 150 minutes per week
Individuals who enroll in the Diabetes Prevention Program will:
- Participate in a one-year Diabetes Prevention Program that includes
- 16 weekly sessions
- Five (5) bi-weekly sessions
- Five (5) monthly sessions
- Self–monitor weight
- Track and learn more about healthy eating and physical activity
- Learn how to problem-solve and stay motivated
