About our CDC-recognized Diabetes Prevention Program

NDPP logoOur lifestyle change program curriculum is part of the National Diabetes Prevention Program, led by the Centers for Disease Control and Prevention (CDC). It is designed for people who have prediabetes or are at risk for type 2 diabetes, but who do not already have diabetes. The lifestyle change program is proven to help people with prediabetes prevent or delay development of type 2 diabetes.

The Program runs for a period of 12 months. This 12-month lifestyle modification program—which is offered online and in-person at Saint Francis Hospital—provides nutritional guidance, exercise sessions, and supports to help prevent or delay diabetes onset. Throughout the program, a trained Lifestyle Coach will give participants the help and support they need to make and sustain lifestyle changes to prevent or delay the onset of type 2 diabetes.

Participants receive tools to help them monitor activity patterns, eating habits, and physical activity to assist them in achieving success. Participation in the program is free to qualified registrants.
