Trinity Health PACE: LIFE Programs
An innovative care program for seniors
Trinity Health PACE offers comprehensive, high-quality care for seniors, allowing them to remain independent while living at home for as long as possible. We address the patient's comfort by providing personalized medical care, in-home support, rehabilitation therapy, prescription drug coverage and specialty services. To further assist with a better quality of life, Trinity Health PACE coordinates all the care and services program participants need. We offer transportation, social services, nutrition planning and spiritual care.
As a Program of All-Inclusive Care for the Elderly (PACE), our holistic approach provides a variety of services, usually at no cost for those eligible for Medicaid or both Medicaid and Medicare. For individuals who don’t qualify for Medicaid, a private pay option is available. In Pennsylvania and Delaware, our PACE programs are called LIFE (Living Independence for the Elderly).




Other available senior services
St. Mary Medical Center’s MG3 Unit is designed to provide comfort and safety for seniors. As a NICHE (Nurses Improving Care for Healthsystem Elders) facility, our specially trained Geriatric Resource Nurses are dedicated to improving health outcomes through a collaborative approach with care management, social work, physical and occupational therapy, the pharmacy and community based providers like LIFE. This facility was designed specifically with seniors in mind and has spacious, private rooms and relaxing colors and décor that feel like home.
Select Specialty Hospital provides long-term care services at Saint Francis Hospital in Wilmington, Delaware. This facility is designed to treat patients with extended care needs. Seniors in need of long-term hospital care often have conditions like heart failure, renal failure, pulmonary disease and non-healing wounds.
Saint Francis Hospital has received accreditation as a Senior-Friendly Emergency Department from The American College of Emergency Physicians (ACEP). Saint Francis is the first hospital in Delaware to receive this certification.
This certification reflects Saint Francis Hospital’s ability to provide the highest quality medical care for older adult patients. It recognizes the hospital’s commitment to providing clinically excellent care, ensuring optimal transitions from the Emergency Department to other settings of care (inpatient, rehabilitation, home care, long-term care), and supporting geriatric-focused quality improvement.
To be accredited, hospitals must be able to provide a set of key services and accommodations for the older adult population, including:
- Geriatric-specific, patient-focused continuing education for all staff
- Additional equipment and renovations focused on the needs of elderly patients
- Case management and social work presence in the ED to assist with care and discharge planning.
