Philadelphia, Pa. — (March 3, 2025) — Nazareth Hospital recently presented a DAISY® Award to Carolyn Smith, RN, a case manager with the Care Coordination department.

Smith was nominated for this prestigious award by Nazareth Hospital’s fifth floor nursing team for providing extraordinary and compassionate care. Smith is “amazing in her role as a case manager each and every weekend that she works,” says the team.

As case manager, Smith is responsible for discharging patients and achieving length of stay goals, including on weekends. According to the Care Coordination team, Smith is meticulous in her work, a pleasure to work with and a clear and effective communicator.

The DAISY Award is a program that recognizes excellence in nursing. It was created by The DAISY Foundation to highlight the clinical skills, extraordinary passion and care exemplified by nurses every day. Award recipients consistently demonstrate each of the award’s criteria of Dedicated, Autonomous, Innovative, Selfless and Extraordinary.

Smith joined Nazareth Hospital in May 2021. She is a kind and caring nurse who is always attentive to her patients’ needs.

Smith’s manager, Edward Cody, BSN, RN, says that Smith is a dedicated team member and deserving recipient of this award. “Carolyn handles even the most challenging cases with ease,” says Cody. “Whenever a patient resists a necessary discharge home or to a skilled nursing facility, Carolyn always comes to the rescue. With confidence and grace, she is able to help patients understand the details of their medical situation, allowing them to make their own decision for a safe discharge process.”
