The Palliative Care Team

Palliative CareThe Palliative Care team is a multidisciplinary group of healthcare providers who are skilled in addressing issues faced by seriously ill patients and their families. Our team includes physicians who are board-certified in palliative medicine and nurse practitioners, who coordinate with the primary care physicians and other specialists to integrate pain and symptom management expertise into the existing treatment plan.

The Palliative Care Team Includes:

  • Palliative care physicians
  • Nurse practitioners or skilled nurse clinicians who help with day-to-day pain and symptom management
  • Social workers who offer counseling services and support
  • Chaplains who can offer spiritual guidance and assistance to patients and their loved ones
  • Case managers who will assist in coordinating care
  • Mercy Home Health services to ensure that patients can continue to receive the same level of palliative care at home

Palliative Care Offers:

  • Improved ability to continue receiving prescribed medical treatments and tolerate them better 
  • A better understanding of the patient’s conditions and choices available for care 
  • Ongoing emotional and spiritual support for patients and their family 
  • Help with symptoms such as pain, shortness of breath, excessive fatigue, nausea, loss of appetite and sleeplessness 
  • Most insurance plans, including Medicare and Medicaid, cover palliative care services. It is handled like other medical specialties, such as oncology or cardiology
