Ambulatory Care Coordinator
Andrea P., Ambulatory Care Coordinator
Andrea is an experienced ambulatory care coordinator at Quality Health Alliance (QHA), where she has been a valuable team member for the past three years. In her role, Andrea supports the care coordination process within the ambulatory care management team, working closely with clinical staff to ensure seamless coordination of care. She plays a key role in assisting with advanced care planning by contacting and scheduling patients to ensure they receive the care and attention they need.
Andrea’s health care career began in 2001 at St. Mary Medical Center in the patient accounting department, where she started in customer service and transitioned into a billing role. In this capacity, Andrea was responsible for managing claims for Aetna, BCBS, commercial insurance, and auto/workman’s compensation. After moving to part-time, Andrea shifted to the emergency department, where she spent 16 years in patient access, eventually rising from registrar to supervisor. Andrea’s broad experience in both administrative and clinical settings has made her an asset in her current role, where she continues to improve patient care and coordination efforts.
Ambulatory Care Nurse Managers
Adrianne (Anne) M., RN, BSN, Ambulatory Care Nurse Manager
Adrianne (Anne) is a dedicated registered nurse care manager for Quality Health Alliance (QHA) at Trinity Health Mid-Atlantic. A 49-year resident of Woodstown, N.J., Anne has spent much of her nursing career providing compassionate care to patients across Pennsylvania, Delaware and New Jersey. With nearly 25 years of nursing experience, her expertise spans oncology, home care and correctional nurse management, giving her a broad and varied perspective on patient care.
Raised by educators, Anne was instilled with the importance of giving back to her community, a value she continues to uphold as a proud mother of three. Her passion for understanding both the medical and social needs of patients, especially within the elderly population, led her to join QHA. Anne’s work focuses on providing comprehensive care that addresses both the health care and emotional needs of her patients, ensuring they receive the highest level of support.
Christina B., RN, CCM, Ambulatory Care Nurse Manager
Christina (Tina) has been a registered nurse at Quality Health Alliance (QHA)for over 8 years, bringing with her a robust 30-year nursing career. Before joining QHA at Trinity Health Mid-Atlantic, Christina worked at Thomas Jefferson University Hospital for 17+ years, specializing in bedside care for women with cancer. In addition to providing direct patient care, she also served as charge nurse for a 30-bed unit and was responsible for creating six-week schedules for the nursing staff.
Tina’s career also includes four years as a case manager at Roxborough Memorial Hospital, where she focused on utilization review. She holds a Bachelor of Science in Nursing and earned her school nurse certification in 2012. In 2019, she achieved certification in case management (CCM), further expanding her expertise in patient care coordination. Tina is committed to a strong work ethic and setting high standards for the delivery of care. Outside of her professional duties, she is an active volunteer at Philadelphia’s largest animal intake shelter, showing her dedication to her community.
Danielle M., RN, BSN, Ambulatory Care Nurse Manager
Danielle is a registered nurse with nearly 20 years of experience. She began her nursing journey at Lower Bucks Hospital as a new RN and later transitioned to St. Mary Medical Center in 2017, where she worked on the telemetry unit. In June 2019, Danielle joined the Quality Health Alliance care management team, where she has continued to make a meaningful impact on patient care. With extensive experience in acute care settings and care management, Danielle brings a wealth of knowledge and dedication to her role.
Kimberly B., RN, BSN, Ambulatory Care Nurse Manager
Kim is a dedicated registered nurse with over a decade of experience in health care, specializing in neuroscience and telemetry. She graduated from the Dixon School of Nursing in 2009 and went on to earn her bachelor’s degree from Immaculata University in 2011. Kim spent 12 years at St. Mary Medical Center, where she worked on a neuroscience telemetry unit, including three years as a clinical lead. During her time in bedside care, Kim developed a strong passion for patient advocacy and quality care, but she was also driven to expand her impact beyond the confines of a 12-hour shift.
Seeking to make a broader difference, Kim transitioned to Quality Health Alliance (QHA), where she could reach more patients and build meaningful, long-lasting relationships within her patient population. In her role at QHA, Kim is able to offer her expertise and support to a larger and more diverse group of patients, ensuring they receive the care and resources they need. Her ability to forge sustained relationships and make a lasting impact on patients’ lives speaks to her dedication and passion for improving health care outcomes.
Mary G., RN, BSN, CNOR, Ambulatory Care Nurse Manager
Mary is a dedicated registered nurse with 22 years of experience in various health care settings. Currently, she serves as a care coordinator at Quality Health Alliance, a position she has held since 2018. Mary’s extensive education includes graduating from Roxborough Memorial Hospital School of Nursing in Philadelphia, Pa., in 2003. She later earned her bachelor’s degree in nursing from Villanova University in Villanova, Pa., and became a certified perioperative nurse (CNOR), further expanding her skill set.
Mary’s nursing career began in 2003 at Albert Einstein Medical Center, where she worked in the surgical trauma intensive care unit as a staff nurse. Throughout her career, she has provided high-level care to a wide range of patients, including those in Level 1 trauma situations, post-operative care for organ transplant patients (kidney and liver transplants), and critically ill post-operative patients. Her experience spans multiple specialties, and she has a long and rewarding history with Lankenau Medical Center, initially starting as a staff nurse in areas such as heart, thoracic, orthopedics, general surgery, transplant, gynecology and robotics. Mary’s leadership abilities were evident as she progressed into roles such as specialty team coordinator for thoracic, vascular, endovascular and plastics services. In this position, she managed nursing staff and worked closely with surgeons and leadership teams to ensure the highest standards of care.
After an impactful 15 years at Lankenau, Mary transitioned into ambulatory care nursing, where she now focuses on coordinating care for high-risk chronic illness patients under Medicare Part A and B. Her role involves collaborating with multiple specialties, both medical and social, to remove barriers to care and ensure her patients lead healthy, happy and safe lives at home. Outside of her professional career, Mary has been happily married for 25 years and is a proud mother to two boys. She values time spent with family, and enjoys sports, outdoor activities and creating lasting memories with her loved ones.
Maya P., RN, BSN, Ambulatory Care Nurse Manager
Maya is an experienced registered nurse with 28 years of nursing expertise. She earned her Bachelor of Science in Nursing (BSN) from Temple University in 1997, graduating in the top 5% of her class. Maya was also a proud member of the National Honor Society and Sigma Theta Tau International Honor Society, reflecting her commitment to excellence in nursing. Before transitioning into her current role as an ACO care coordinator, Maya dedicated over 24 years to providing home care services, working both as a clinical nurse and case manager. She has a strong passion for working with vulnerable geriatric populations, particularly those from diverse, low-income Medicare/Medicaid communities.
Maya has played a pivotal role in facilitating safe transitions of care and connecting patients to essential resources, helping to reduce unnecessary hospital readmissions and improve patient outcomes. In addition to her extensive clinical experience, Maya is multilingual, and fluent in English, Russian and Ukrainian, allowing her to better serve a diverse patient population and build stronger connections with her patients and their families.
Robin Y., BSN-RN, ACM-RN, Ambulatory Care Nurse Manager
Robin began her nursing career at Crawford Long Hospital in Atlanta, Ga., working on a cardiothoracic step-down unit. After relocating to Pennsylvania, she joined St. Mary Medical Center in 2007, where she worked on the progressive care unit. In 2008, Robin transitioned to St. Mary Home Care as a visiting nurse, later returning to the hospital as a discharge planner/liaison for St. Mary Home Care.
In 2013, Robin worked for a company contracted by St. Mary Medical Center, focusing on heart failure patients in the community. This role eventually led her back to the hospital in 2014, where she worked with heart failure patients as part of a Medicare bundle payment program. Her work in this area contributed to the creation of the Accountable Care Organization (ACO), Quality Health Alliance, where Robin now serves as an ambulatory care nurse manager. In October 2020, Robin earned her Accredited Case Manager (ACM-RN) certification. She holds an RN license in both Pennsylvania and New Jersey.
Stacey S., RN, BSN, BSW, CCM, Ambulatory Care Nurse Manager
Stacey is an experienced registered nurse, social worker and certified case manager with a diverse background spanning over 15 years in health care. Her career includes experience in acute care settings such as the emergency department, ICU and peri-operative care, as well as school nursing, home and hospice care, and utilization review. Stacey holds both a Bachelor of Science in Nursing from Thomas Jefferson University and a Bachelor of Social Work from Cabrini University, which allows her to combine clinical expertise with a patient-centered, holistic approach to care.
With extensive experience working with diverse patient populations—from pediatrics to seniors—Stacey has spent over 14 years collaborating with major insurance companies, private medical practices and university hospital systems to enhance the patient and family experience. Currently, Stacey serves as an ambulatory care nurse manager with Quality Health Alliance, specializing in transitional care management for the Managed Medicare population. In her role, she works to improve care adherence, reduce hospital admissions, address social determinants of health, and provide patient education tailored to personal health goals. She is dedicated to working with the interdisciplinary team to streamline the discharge process, coordinate home care and community resources, and implement disease management programs. Stacey is passionate about optimizing health resources by addressing social barriers and advocating for her patients and families. Outside of work, Stacey enjoys attending her sons’ sporting events and educational pursuits, staying active with walks and exercise, and continuing her education to stay up-to-date with the evolving health care landscape.
Tracey R., RN, BSN, MHA, Ambulatory Care Nurse Manager
Tracey has a diverse career in nursing, with over 30 years of experience across various roles and settings. Her academic background, including a BSN from the University of Pennsylvania, an MHA from Drexel University, and certification as a case manager, reflects a solid foundation in both clinical practice and health care management. Her career trajectory showcases a wide range of expertise, particularly in cardiac nursing and case management, having worked in prestigious institutions like the Hospital of the University of Pennsylvania. Her roles have spanned across multiple disciplines, such as community liaison, SIDS counseling and leadership in assisted living and home care. These experiences suggest a deep understanding of patient needs in both acute and chronic care settings, as well as in-home and community environments.
Tracey’s extensive work in case management, particularly with home care and at organizations like Mercy LIFE and the Visiting Nurse Association (VNA) of Philadelphia, highlights her ability to coordinate complex care plans and navigate the intricate healthcare system for vulnerable populations. As a nurse case manager with Trinity Health and the ACO for seven years, and now with the Keystone 65 HMO population, her work has likely focused on ensuring patients receive appropriate care, advocating for their needs, and improving outcomes, especially for those in managed care systems.
Victoria J., RN BSN, Ambulatory Care Nurse Manager
Victoria is a dedicated care manager at Quality Health Alliance (QHA) at Trinity Health Mid-Atlantic, where she has worked since 2018. She is passionate about providing holistic care to her patients, working collaboratively with a multidisciplinary team that includes social workers, health coaches, pharmacists, RNs, directors and medical directors. Victoria believes that this strong coordination of care leads to better patient outcomes, enhanced care and lower costs. She values the teamwork and support that Trinity Health Mid-Atlantic provides and has never worked with a more dedicated team.
Before joining QHA, Victoria worked at Christiana Care from 1996 to 2018. During her time there, she held multiple roles in care management, including as a care coordinator and preceptor from 2012 to 2018. Prior to her work in care management, Victoria worked as a registered nurse in the Christiana Care emergency department from 1997 to 2012 and in the medical step down/ventilator unit from 1996 to 1997. Victoria’s career began in 1992 at Memorial Hospital of Salem County, where she worked as a registered nurse in the intensive care unit until 1996.
Catherine T., RN, BSN, Ambulatory Care Nurse Manager, Heart Failure Program
Catherine is an experienced care manager for Quality Health Alliance (QHA) at Trinity Health Mid-Atlantic. A long-time resident of Kennett Square, Pa., Catherine has spent much of her nursing career serving patients in Delaware. She was one of the original care managers hired when the Accountable Care Organization for Saint Francis in Wilmington, Del., was established in 2016.
Catherine earned her nursing diploma from The Baptist Memorial School of Nursing in Memphis, Tenn., in 1985 and later received her Bachelor of Science in Nursing (BSN) from Immaculata University in Malvern, Pa., in 2012. With nearly 40 years of experience, Catherine’s career has included bedside nursing, home care and leadership roles. She has supervised the care management department for Aetna’s former Medicaid company in Delaware (Delaware Physician’s Care) and oversaw the education and management of medical assistants at a Federally Qualified Health Center (FQHC).
Raised as the daughter of a foreign service officer, Catherine developed fluency in Spanish, which has been invaluable in her work with non-English speaking patients. She also volunteers for the predominantly Mexican population in her community, further demonstrating her commitment to serving diverse populations.
Denise M. W., RN, BSN, ACM, MEd, Ambulatory Care Nurse Manager, Heart Failure Program
Denise is an accomplished registered nurse with a diverse career in health care, spanning several specialties. She began her nursing career in cardiology, working in the telemetry and coronary intensive care unit (CICU) at Albert Einstein Medical Center. After gaining valuable experience, she worked part-time visiting patients for Hospice of The VNA of Philadelphia. Denise then transitioned to the School District of Philadelphia, where she became a certified school nurse for grades K-12. During this time, Denise pursued her master’s in counseling psychology from Temple University and later served as a certified secondary school counselor for eight years within the district.
In 2014, she returned to her community nursing roots as a visiting nurse care manager for a research and development company, focusing on a congestive heart failure (CHF) bundle care program in collaboration with Medicare and St. Mary Medical Center, where she provided intensive support to heart failure patients in the community. Denise’s career continued to evolve, and in 2016, she became a community health nurse at St. Mary Medical Center. This role eventually led her to her current position as an ambulatory care nurse manager with Quality Health Alliance (QHA), where she utilizes her extensive cardiology experience to support heart failure patients. In 2020, Denise earned her Accredited Case Manager (ACM) certification and holds RN licenses in both Pennsylvania and New Jersey.
Behavioral Health Specialists
Anne A., LCSW, Behavioral Health Specialist
Anne A. brings over 30 years of experience in the field of behavioral health, with extensive expertise in both inpatient and outpatient settings. Her career has spanned across a wide range of roles, each focused on providing critical support to individuals facing mental health challenges. Prior to joining Trinity Health Mid-Atlantic, Anne served as the director of social services on the inpatient adult psychiatric unit at the Kirkbride Center, where she was responsible for overseeing social work services in a highly demanding and complex care environment.
Anne’s experience also includes significant leadership roles in the development and implementation of social work programs, particularly in inpatient dual diagnosis units at the Kirkbride Center and Eugenia Hospital. Her work in these settings reflects her ability to handle the complexities of co-occurring mental health and substance use disorders, ensuring that patients receive integrated and comprehensive care. In addition to her clinical roles, Anne served as site supervisor at PATH, Inc., where she provided crisis intervention and counseling for adults with mental illness, as well as overseeing a residential rehabilitation program for individuals with chronic mental illness. This role required a strong ability to manage both clinical and administrative duties while delivering vital services to some of the most vulnerable populations. Anne holds the credential of licensed clinical social worker (LCSW), which underscores her expertise in mental health and social work practice. With her deep knowledge of behavioral health systems, her leadership in creating innovative care models, and her extensive direct service experience, Anne is a highly skilled and compassionate professional in the field of mental health care.
Maggie Walsh, LCSW, Behavioral Health Specialist
Maggie is a highly experienced licensed clinical social worker, psychotherapist and educator with over 20 years of expertise in behavioral health and substance use. Throughout her career, Maggie has worked in a variety of clinical settings, developing a diverse and compassionate approach to mental health care. In her previous role as director of social services in a psychiatric hospital, Maggie confronted the challenges of high-stress environments firsthand. This experience led her on a personal journey to explore the transformative potential of meditation, which she ultimately integrated into her therapeutic practice. This unique combination of clinical expertise and mindfulness practices has allowed Maggie to help her patients find emotional balance and healing, especially in managing stress, anxiety and depression.
Maggie holds multiple certifications, including in behavioral health and gerontology, sexual assault counseling and as a trauma specialist. Her certification in Mindfulness-Based Stress Reduction (MBSR) is a cornerstone of her therapeutic approach, enabling her to guide patients in cultivating peace and clarity. By integrating evidence-based practices (EBP) like cognitive behavioral therapy (CBT) with mindfulness techniques such as relaxation, guided meditation and MBSR, Maggie helps her patients develop essential coping skills, enhance emotional regulation and improve their overall mental well-being. For Maggie, her work as a behavioral health social worker is much more than just a career—it’s a calling. She is deeply committed to people-centered care, offering both stewardship and service to others. With her unique blend of clinical knowledge and mindfulness practices, Maggie empowers her patients to cultivate resilience and healing in the face of life’s challenges, helping them achieve lasting emotional health.
Clinically Integrated Network Managers
Aileen W., MBA, CIN Manager
Aileen is a dedicated Clinically Integrated Network (CIN) Manager for the Langhorne market around St. Mary Medical Center, working with Quality Health Alliance (QHA). She earned her Master of Business Administration (MBA) from West Chester University and joined the QHA team in April 2022. Aileen has been an integral part of the St. Mary family since 2008, starting as a medical assistant before transitioning into wellness and quality transformation roles. She later served as the quality manager for the medical group, further honing her expertise in health care management and quality improvement.
In her current role as CIN Manager, Aileen takes pride in supporting Trinity Health affiliate and owned providers as they transition to value-based care models. Her focus is on improving the health and quality of entire populations, working to ensure that care is both efficient and effective across the board. Outside of her professional life, Aileen enjoys spending time with family and friends, traveling to the Caribbean, baking and staying active through exercise.
Antoinette R., RN, BSN, CIN Manager
Antoinette is the Clinically Integrated Network (CIN) Manager for Philadelphia and the surrounding counties. She earned her Bachelor of Science in Nursing (BSN) from Holy Family University and joined the Quality Health Alliance (QHA) team in September 2023. With over 17 years of experience in primary care, Antoinette began her career as a medical assistant, working for 9 years before pursuing her nursing degree. Antoinette’s career includes six years as a Wellness Coordinator for a medical group, followed by a role as quality manager in 2022. Her diverse experience in quality assurance, nursing and administration has equipped her with the perfect foundation for managing a network of providers focused on population health.
In her current role, Antoinette supports practices by providing resources to enhance quality outcomes, improve patient care, and aid in care coordination efforts. Outside of work, Antoinette enjoys spending time with her family, especially participating in outdoor activities and visiting places like the Philadelphia Zoo and Sesame Place to create joyful experiences for her children.
Kevin C., BS, CIN Manager
Kevin holds a bachelor’s degree in allied health from Temple University and joined Quality Health Alliance at Trinity Health Mid-Atlantic in April 2022. With over 25 years of experience in the health care industry, Kevin brings a wealth of knowledge to his role, where he supports participating physicians in the Wilmington community as they work on practice transformation initiatives. Outside of his professional responsibilities, Kevin enjoys football, swimming and spending quality time with family and friends.
Community Health Worker
Bri’Ann P., CHW
Bri’Ann is a dedicated Community Health Worker (CHW) whose journey into the field began with a deep passion for helping others navigate life’s challenges. Over the years, she has worked in various roles within the community, advocating for and guiding families toward essential resources. Through her experiences, Bri’Ann saw firsthand how access to crucial resources can dramatically improve lives, sparking her desire to formalize her training as a community health worker.
Bri’Ann’s work goes beyond simply providing resources; it’s about empowering individuals, building trust, and bridging the gaps between communities and health and social services. She is committed to ensuring that those she serves not only receive the care they need but also gain the confidence and support to take control of their own health and well-being.
Pharmacist
Jeffrey G., PharmD, MS, BCPS, CDCES, PDE-C, HF-Cert, DPLA
Jeff received his Doctor of Pharmacy degree from the University of the Sciences in Philadelphia in 2008. He received his Master of Science in Pharmacy with a concentration in medication therapy management from the University of Florida in 2022. He is recognized as a board-certified pharmacotherapy specialist from the Board of Pharmacy Specialties, a certified diabetes care and education specialist from the Association of Diabetes Care and Education Specialists, a certified pulmonary disease educator from the American Association of Respiratory Care, and certified in care of heart failure patients by the Heart Failure Society of America.
Upon graduating from the American Society of Health-System Pharmacy’s Pharmacy Leadership Academy in 2024, he was recognized as a Diplomate of the Pharmacy Leadership Academy. His practice has crossed several sectors of health care including community, acute care and ambulatory pharmacy.
Post-Acute Care Liaison
Susan R., MPT, BSN, CCM, Post-Acute Care Liaison
Susan is an accomplished health care professional with a diverse background spanning physical therapy, nursing and case management. She graduated from Drexel University in 1998 with a master’s in physical therapy and has worked across various settings, including acute rehabilitation, skilled nursing facilities (SNFs), home care and neurological rehabilitation. Susan also served as the regional director for a group of seven SNFs, where she gained valuable leadership experience.
In 2006, Susan earned her Bachelor of Science in Nursing (BSN) from LaSalle University and joined the Penn Health System, where she obtained her Certified Case Manager (CCM) certification. She went on to lead the case management/social work department at Penn and pursued a master’s in nursing at the University of Pennsylvania, further honing her skills in patient care and management. Additionally, Susan worked as a project manager for a startup company focused on Case Management Software, gaining expertise in health care technology and software implementation. Currently, Susan is back in the post-acute care environment with Trinity Health Mid-Atlantic, serving as a liaison. In this role, she focuses on optimizing SNF length of stay and building strong relationships with SNFs in the community to ensure high-quality patient care and seamless transitions.
Social Workers
Bethany H., LSW, Ambulatory Care Social Worker
Bethany is a licensed social worker with over 10 years of experience in the health care field, all of which have been with Trinity Health Mid-Atlantic. She began her career at St. Mary Medical Center as an acute care social worker, where she developed a strong foundation in providing support for patients in critical care. Following this, Bethany served as the community liaison for LIFE St. Mary, a program of all-inclusive care for the elderly, where she worked to enhance services for senior patients and connect them with vital resources.
Currently, Bethany serves as an ambulatory care social worker with the population health team, where she continues her commitment to improving patient outcomes. Her work focuses on supporting patients in outpatient settings, ensuring they have access to the resources and care needed to manage their health outside of the hospital.
Jasmine W., MSW, Social Worker
Jasmine is a dedicated master-level social worker specializing in gerontology, bringing a wealth of experience to her role at Quality Health Alliance (QHA). With a deep passion for supporting vulnerable populations, she collaborates closely with nurse case managers to ensure that patients, particularly older adults, access critical resources that enhance their health and well-being. Her expertise spans a wide range of areas, including nutrition, financial assistance, medical education and facilitating access to Medicaid, durable medical equipment (DME), and waiver programs.
Joining QHA was a natural progression for Jasmine, driven by her desire to empower patients and their families to successfully navigate the health care system. She understands the complexities and emotional toll that come with managing chronic medical conditions, coordinating care and ensuring access to essential services. Through her work, Jasmine aims to reduce hospital readmissions and help patients feel more in control of their healthcare journey. A key area of Jasmine’s work is advanced care planning. She supports patients and families in making informed decisions about their long-term care by guiding them through advance care directives and goal setting. Jasmine meets patients where they are, ensuring their preferences are respected and their health care needs are met with dignity and compassion. Her holistic approach to care, commitment to patient advocacy, and ability to bridge gaps in the health care system make Jasmine an invaluable resource to the individuals and families she serves.
Kimberly W., LMSW, MSW, BA, Social Worker
Kim began her career at the District of Columbia Medicaid/Food Stamps office, helping clients access critical benefits. She later transitioned to Children’s National Medical Center as a family services associate, where she assisted social workers across various units. After obtaining her master’s in social work from Delaware State University, Kim shifted her focus to home health care, specifically serving the geriatric population, a field she has been dedicated to for 16 years. During this time, Kim developed expertise in assessing complex situations, crafting interventions and advocating for her patients, always working in collaboration with interdisciplinary teams to provide comprehensive care. Kim believes in the power of human connection, striving to build strong, trusting relationships with both patients and their caregivers. Her commitment to the geriatric population is driven by a deep sense of fulfillment and purpose, especially in supporting those navigating the complexities of aging.
In October 2021, Kim's life took a transformative turn when she was involved in a tragic car accident that left her paralyzed from the waist down. This life-altering event allowed her to step into a new role at Quality Health Alliance, where she continues her work with the geriatric population. The accident has deepened her empathy, giving her a personal understanding of the challenges some of her patients face, from transportation struggles to emotional difficulties and everyday life concerns. Kim’s personal journey has only enhanced her ability to relate to her patients, and she openly shares her story, fostering a deep sense of connection and trust with those she serves. Her patients often express gratitude for her empathy and the shared understanding of their struggles and accomplishments. Kim’s unwavering dedication to improving the lives of older adults and her ability to foster meaningful connections make her a vital asset to the health care community and a cherished advocate for her patients.
