Seniors who live at home with health problems need special care. Without it, they may return to the hospital again and again.
“A recent study shows that our program can cut hospital visits for frail seniors by more than 40 percent,” says Donna Raziano, MD, medical director of Mercy LIFE. “That’s because we plan our patients’ care as a team. We know each patient well. If there is a problem, we all help out.”
Home From the Hospital
“It can be stressful for patients and their families to come home from the hospital with a pile of prescriptions and a complicated care plan,” says Dr. Raziano.
At LIFE, a nurse can set up the pillbox and make sure patients understand how to take the medication. If needed, LIFE staff will order a ride from the hospital and a hospital bed for the home. LIFE schedules doctors’ appointments and home visits from healthcare experts. “We make the arrangements for when patients return home from the hospital and then we follow up,” explains Dr. Raziano. “That way, there is less possibility of a setback.”
