Nazareth Hospital Appoints Orthopedic Nurse Navigator

NORTHEAST PHILADELPHIA (July 28, 2015): Nazareth Hospital recently appointed Brittany Sharp, BSN, RN, as its Orthopedic Nurse Navigator. Brittany has more than a decade of healthcare experience, with roles in medical-surgical and care transition nursing.

In her role, Brittany facilitates the healthcare experience for our patients who need orthopedic care. Brittany’s focus is ensuring continuity of care for patients and their families, especially for those patients with a higher risk of re-hospitalization. Brittany earned her bachelor’s degree in nursing from Holy Family University.

Brittany assists patients as an Orthopedic Nurse Navigator in some of the following ways:

  • Helps patients line up testing, pre-surgery classes, appointments and follow-up to ensure continuity of care 
  • Educates patients and their families about their conditions, their procedures and their care
  • Answers questions 
  • Offers empathy and support
  • Assesses patients’ needs and connects them with needed resources, such as home care, to ensure optimal outcomes

Nazareth Hospital’s orthopedic program includes a state-of-the-art Center for Physical Therapy, Rehabilitation and Balance; nationally accredited Acute Rehabilitation Program for complex inpatient rehabilitation needs; and a Transitional Care Unit for patients who need additional therapy before returning home. Nazareth Hospital is also nationally accredited by The Joint Commission for hip and knee replacement.
